What should I do to prepare for my first visit?
In preparation for your child’s evaluation:

  • Obtain a prescription from your pediatrician for evaluation and treatment sessions.
  • Complete and print In-Take form from our website.
  • Review the Attendance and Procedure policy. You will be required to sign this on the first day of treatment.
  • Verify with your insurance carriers regarding visit limits, co-pays or deductibles, and if pre authorization for visits are required.
  • Have insurance cards with you so that we can make a copy of them.
  • Bring with you any evaluations or documents that you feel may be important for the therapist to review.

What will happen during my first visit?
In preparation for your child’s evaluation:

  1. If you have not already filled out the required forms, you will be asked to complete them at the time of the visit.
  2. Depending on your child’s age and reason for referral, you may be asked to fill out additional parent questionnaires.
  3. Please dress your child comfortably.
  4. You will be expected to pay the private pay fee or any copays at the visit. See Insurance & Payment for more information.
  5. Please allow a full hour for the initial appointment.

Referral Process and Insurance Policies
Patients may be self-referred, referred by physicians or by other health care professionals. We are an in-network provider for many insurance companies in the state of Pennsylvania. Pre-authorization and filing of insurance are services provided for patients by our clinic. In addition, we provide services through Carbon-Monroe-Pike MHDS, we are a provider for Early Intervention Services for Carbon-Monroe-Pike counties of Pennsylvania.

Inclement Weather and Holidays
Our clinic times do not necessarily coincide with private or public school openings and closings due to weather. We will call you to cancel sessions if our offices are closed. Please call our main office if you are unable to attend your scheduled session due to hazardous driving conditions. Keep in mind that often time schools are closed while the roads nearer our offices are quite clear, but NEVER jeopardize your safety due to poor driving conditions. Just give us a call.

Cancellations
If you must cancel an appointment, we request that this be done at least 4 hours prior to the scheduled appointment time. TGTS LLC Therapist’s will plan with you to effectively utilize your authorized insurance sessions. It is the responsibility of the client to schedule the sessions within this time frame.
